Encourage reporting without fear

If people worry about blame or punishment, they won't report. Make it clear that the goal is to learn and improve, not to discipline. Support anonymous or confidential reporting where it helps, and thank people who report. Leadership should consistently reinforce that reporting is valued.

Simplify the process
